Your privacy, plainly
Huntsville Smiles Dentistry is a health information custodian under Ontario’s Personal Health Information Protection Act, 2004 (PHIPA). We collect, use and share your personal health information only to provide and support your care, and only as the law allows.
What we collect
To care for you we keep your contact details, health and dental history, x-rays and clinical notes, treatment records, and insurance and billing information.
This website
When you send an appointment request or message through this site, we receive the details you enter (your name, phone, email and the reasons you choose) and use them only to contact you about your request. Please don’t include detailed health information in web forms or email. We’ll ask in person.
The site doesn’t use advertising cookies. Your browser may store small preferences, such as whether the menu is open, on your own device.
Sharing
We share information only with your consent, or as permitted or required by law. For example, we may share it with your insurer to process a claim you’ve asked us to submit, with another health care provider involved in your care, or with a dental laboratory making a crown or denture for you.
Your rights

Records from Dr. Litchfield’s practice
Patient records from Dr. Doug Litchfield Dentistry remain at this office and continue to be protected under the same law.
